INFORMATION FOR TERMINATION OF EMPLOYMENT
(non-regular, wage, FT2, postdocs)

For non-regular (wage, FT2, postdocs) personnel leaving the Penn State College of Earth and Mineral Sciences Check List (revised )

Upon termination, you may have questions regarding your last paycheck, continuation of benefits, return of University keys, parking permits, and so forth. To assist you with these inquiries, the following information is provided (also, refer to HR-55 of the PSU Policy Manual)

1. EMPLOYEE BENEFITS-CONTACT EMPLOYEE BENEFITS DIVISION (865-1473) if you are enrolled in PSU health insurance coverage.

The University's group health insurance program (Medical) generally covers the staff member until the end of the pay period in which the termination is effective. Postdoctoral scholars who have prepaid for insurance beyond their termination date should contact Employee Benefits to request a reimbursement. Note that reimbursement will be adjusted for any rate increases that have occurred.

You may be eligible for continuation of health care benefits under the Consolidated Omnibus Budget Reconciliation Act ( COBRA ) . After 18 months of COBRA coverage, you may contact HealthAmerica about a conversion policy. If interested, contact HealthAmerica Member Services Department, 814-231-8970 or write to them at 1850 East Park Avenue, State College, PA 16803.

2. RETIREMENT CONTRIBUTIONS- for part-time employees (wage or FT2) who are members of SERS or TIAA.

    SERS (State Employees Retirement System) -Contact your SERS counseling center by calling 863-6505 or, outside the State College area, call the toll free number 1-800-633-5461 . SERS requires that any person, non-vested as well as vested, who terminates from Penn State be advised to contact his or her counseling center. Penn State's Employee Benefits Division no longer provides the application for refund of member's contributions and interest.

    TIAA (Teachers Insurance and Annuity Association) -Write to TIAA at 730 Third Avenue, New York, NY, 10017-3206 (toll free: 1-800-842-2776)

3. CHANGE OF ADDRESS -If you change your address after your termination, or have previously changed your address without reporting it, a new W-4 card must be on file to ensure that you receive your W-2 Form for income tax purposes. If you have not received your W-2 Form by January 31, you will need to contact the Payroll Office at 814-865-7621.

4. Eligibility for UNEMPLOYMENT COMPENSATION BENEFITS is a responsibility of the Office of Employment Security. Please direct inquiries regarding that benefit to the local Office of Employment Security in the area in which you reside.

5. Arrangements must be made with the Bursar's Office to have your FINAL PAYCHECK mailed to you, if your usual method of paycheck receipt is inappropriate . Either go to the Bursar's Office in 103 Shields Building and complete the required form or call 814-865-6528 and ask for the Payroll Area. If you currently have your paycheck automatically deposited, you must first file a new "Salary Deposit Request Card" to stop automatic deposit of your last check. You may obtain these cards in 116 Deike Building, 315 Rider Building, or from your department office.

6. RETURN ALL UNIVERSITY PROPERTY

Contact the EMS Dean's Office, 116 Deike Building, 814-865-6546, to return equipment , parking permits , and keys (you will receive a full refund on key deposits when keys are returned.).

Return all other University property to the appropriate office(s), e.g., American Express Card, telephone credit card, library books, and so forth.

7. SAFETY -If it applies to your situation, assure that all containers of chemicals in your control are clearly labeled as to the contents, and all chemical waste is properly manifested to the Office of Environmental Health and Safety.

8. EARNED TIME PAY - Staff wage employees be sure to give your earned-time record card to your supervisor. You will receive the cash value of unused earned time provided you have given at least two weeks advance notice of your termination.
